- Procedures for Requesting Disclosure
Requests for "notice of purpose of use", "disclosure", "correction, addition, or deletion of content", or "suspension of use or erasing" relating to personal data held by our company from the principal or the principal's representative can be made by filling out the required information in the request form prescribed in 1 below, enclosing the necessary documents prescribed in 2 below, and mailing it to the address prescribed in 3 below. Please understand that we cannot accept any other method of request.

2. Documents, etc. to be enclosed
(1) Identification documents
- In the case of request by the principal
Enclose any two of the following items that include the current address.
- Copy of driver's license
- Copy of passport
- Copy of health insurance card
- Certificate of residence
- Seal registration certificate
- Copy of pension book
- Copy of basic resident registration card
- Copy of alien registration certificate
- In the case of request by representative
In addition to the above identification documents, enclose one of the following items.
- In the case of a statutory representative of a minor: Copy of the family register
- In the case of a statutory representative of a minor: Certificate of registered matters pertaining to the registration of adult guardianship
- In the case of a lawyer: Power of attorney
- In the case of an authorized representative: Power of attorney (with the registered seal of the principal affixed), seal registration certificate of the principal
(2) A reply envelope with 800 yen stamp(s) attached
If the request is for disclosure of retained personal data or notice of purpose of use, write down the claimant address on an envelope, attach stamp(s) worth 800 yen to it (for registered postage with proof of delivery), and enclose it. (If the request is for correction, addition, deletion, suspension of use, or erasing of retained personal data, it is not necessary to enclose a reply envelope)
